
Basic Street Outreach drop in center program will identify and establish relationships with at-risk
youth runaways, homeless and street dependent youth ages 13-18. This program will screen and
assess 20 at-risk youth runaways, homeless and street dependent youths and bring them off the
streets. This program will engage youths with activities at drop in centers while encouraging
them to come off the streets and into shelters throughout Polk county. Through partnerships and
collaborations this program will provide/link continuum of care for safe, stable long-term
housing, coordinated targeted case management, behavioral health services, connection with
family and community through mentors and volunteer activities. Provide life skills and link to
education/trade and tutoring services, and employment opportunities for these youths to become
independent, self-sufficient and contributing members of our communities. Provide follow-up
services to ensure the youths are independent, self-sufficient, living risk free lives and remaining
off the streets.
